USMLE Application Process

Every time a student applies for an eligibility period to sit for the USMLE Step 1, Step 2CS and Step 2CK,
Saint James School of Medicine must certify the student’s status with the school before ECFMG will
approve them. SJSM has established a unique set of requirements for each USMLE exam the student must
meet before the school will complete the certification process.

NBME Comprehensive Basic Science Exam Prequalification Requirement
All students are required to pass the NBME Comprehensive Basic Science Exam (CBSE) as a
prequalification requirement. Students may attempt the NBME CBSE as many times as necessary to pass and
they will be required to take and pass the NBME every time they are certified for Step 1.

Eligibility for NBME CBSE

  • Must successfully complete the fifth semester.
  • Must be in good academic and financial standing.
  • If the student has previously failed an attempt of the NBME CBSE or the USMLE Step 1, they must complete an academic counseling session with a member of the Office of the Dean of Clinical Science before retaking the exam.

Passing Scores for NBME CBSE

  • The passing score will be based on the equivalent USMLE Step 1 score provided on the score report from NBME.
  • A higher passing score is required for the third and fourth time a student applies for the USMLE. It IS NOT based on the number of NBME attempts.
    • First and Second Certifications: ≥192 USMLE Step 1 equivalent
      • This is currently a 67 on the NBME CBSE; however this is subject to change without notice based on tables provided by NBME. SJSM does not accept scores within the standard error of measurement.
    • Third and Fourth Certifications: ≥207 USMLE Step 1 equivalent
      • This is currently a 73 on the NBME CBSE; however this is subject to change without notice based on tables provided by NBME. SJSM does not accept scores within the standard error of measurement.

Academic and Financial Standing
Must be in good academic and financial standing at the time they are certified for Step 1.

NEED TO KNOW: Any student who does not pass Step 1 after their fourth certification will be
dismissed even if the student did not sit for the exam after being certified.

USMLE Step 2 CS and Step 2 CK Requirements
Graduation Paper Proposal
All students are required to submit their graduation paper proposal prior to being certified for whichever Step
2 exam they are taking first.

Completions of Core Rotations
All students are required to complete all six core rotations before either Step 2 application is approved by SJSM.

NBME Comprehensive Clinical Science Exam Prequalification Requirement (ONLY Step 2 CK)

All students are required to pass the NBME Comprehensive Clinical Science Exam (CCSE) as a
prequalification requirement. Students must retake and pass the NBME CCSE every time they are certified
for Step 2 CK. There is no prequalification exam required for Step 2 CS.

Eligibility for NBME CCSE

  • Complete all six core rotations.
    • Can select a testing window that ENDS up to 30 days prior to the last day of the last core rotation.
  • Must be in good academic and financial standing.

Passing Scores for NBME CCSE

  • For the NBME CCSE, the passing score will be based on the equivalent USMLE Step 2 CK score provided on the NBME score report.
  • For all certifications, must score ≥209 USMLE Step 2 CK equivalent.
    • This is currently a 74 on the NBME CCSE; however this is subject to change without notice based on tables provided by NBME. SJSM does not accept scores within the standard error of measurement.

Academic and Financial Standing
Must be in good academic and financial standing at the time they are certified for Step 2 CK and CS.

How to Apply for the USMLE Exams

First Application
For first time applicants only, SJSM must receive the Step 1 application from ECFMG within 45 days of receipt of a passing NBME score. If the application is not received by this time, the student will be required to retake and pass the NBME when they are ready to complete their Step 1 application.

Below is a brief summary of the application process. For more information must visit the ECFMG website or contact them directly.

Process for Applying for the Step 1 Exam:

  • Go to ECFMG website (www.ecfmg.org).
  • Create profile.
  • Get USMLE ID number.
  • Fill out online form Interactive Web Application (IWA).
  • Select “eligibility period”.
  • Pay appropriate application fees to ECFMG.
  • Print Form 186 and have it signed and stamped by notary or other appropriate official.
  • Sign completed Form 186 directly to ECFMG.
  • ECFMG will take three to five weeks to process Form 186.
  • ECFMG will electronically send application to SJSM for status verification.
  • SJSM will complete status verification once all stated requirements are met.
  • ECFMG will send final approval and eligibility period to student.

All Other Applications
The Form 186 is valid for five years. Applicants with a valid Form 186 can apply for exams or request
eligibility period extensions by completing only the online part of the application.

For all students applying for Step 1 for the second, third, or fourth time; and all students applying for any Step 2 exam, SJSM must receive the online application from ECFMG within 30 days of passing a prequalification exam. If the application is not received by this time, the student will be required to retake and pass the NBME when they are ready to complete their Step 1 application.

Save All Score Reports

Students are required to send/email/fax in score reports for all attempts for all USMLE exams. Students will
not be allowed to schedule rotations until a passing Step 1 score report is received by the school. Step 2 CS
and Step 2CK reports must also be provided as they are graduation requirements.
